Whether you are living with Alzheimer’s disease or another form of dementia or are supporting someone who is, the Alzheimer Society of York Region’s First Link® program can connect you to community services and the supports you need.
Empowerment is the aim of this program, enabling families and their loved ones to make informed choices so that they can live a more fulfilling life with dementia—at every stage.
The Western York Region Ontario Health Team is pleased to support this program, connecting primary care physicians with the tools to support individuals with navigating the complex world of Alzheimer’s and dementia care.
With First Link®, participants receive free access to ongoing individual support, educational resources, links to local community services, and the opportunity to lean on the support of others through shared experiences.
If you are interested in this program, contact your family physician and ask to be referred to the First Link program. Once the referral has been made, the First Link® Coordinator will contact and connect you to available supports and educational programs.
